If you are making an appointment, please inform us of the reason of your visit so we can allocate the proper amount of time for your visit. Our office visits are by appointment only. If you have an acute problem, we will try to accommodate you the same day.

If this is your first visit... please arrive 15 minutes early to fill out the medical history and registration forms. Have your medical insurance card and any important medical records with you. Please inform us about any health problems, allergies and any medications you are taking.
